Effective for the 2011/2012 season MDMHA has moved to an early registration system. We strongly recommend that you register at the banquet (date TBD).
In person registration will occur at the Millbrook Lion's Den ONE day only - Wednesday June 15, 2011 6:00 pm - 9:00 pm.
Registrations received by June 25, 2011 will be guaranteed a place on a team for the 2011/2012 season.
Families registering more than two children with MDMHA shall receive a $25 discount per child starting with the third child.
Late Registrations received after June 25, 2011 are subject to a $50.00 late fee and will be placed on a team if team numbers allow. Team sizes and registration numbers may dictate late registrations being added to a 'wait list'.
Players planning to try out in other centers must register (including tryouts) with MDMHA by June 15/2011 to be guaranteed a place on a Millbrook team. Contact the OMHA Rep to obtain permission to try out in another center. Players must be registered (including tryouts) with MDMHA to be eligible for a release to another centre in the event there is no Rep team in their age group.
Payment of total registration fees may be divided into 6 equal post-dated cheques payable to Millbrook & District Minor Hockey Association (MDMHA) and dated: April 1, May 1, June 1, July 1, August 1 and September 1. Cheques not dated the 1st of the month will be held onto and cashed the following 1st of the month.
